Endocrinologist

LiveWell Endocrinology will follow the direct care medicine model which allows the practice to run without the involvement of third-party insurance companies. We believe this approach will offer our patients a vastly superior experience.

Visits and services will be cash pay; however, we will submit claims directly to insurance providers for out-of-network reimbursement. Please reach out to our office and we can help provide an estimate of costs. You can also submit your information to us so we can help verify your benefits through Reimbursify.

Payment is required in full at the time of service. Lab tests, imaging studies, and medication prescriptions will still be processed through insurance.

Please note that we do not work with any federal or state-funded third-party payors. Therefore, no reimbursements for our services will be provided by governmental third-party payors.

Nutritionist

At LiveWell Endocrinology, we have a nutritionist that can create a meal plan or arrange a delivery service with attention to your metabolic needs as outlined by our team. Our nutritionist will offer consults in-person and virtually to fit your needs and schedule.

Rates:

New patient evaluation: $225, Follow up visit: $125, Visit to grocery store: $150

Personal Trainer

We will help arrange personal training sessions which are also offered based on the trainer’s prevailing rates. Patients will be able to get package rates that include multiple sessions or they can book each session individually.

If you attend a visit with any of our endocrinologists, you are entitled to one free 30-minute session with our preferred trainer.

Missed Appointments / Late Cancellations

At LiveWell Endocrinology, we greatly value our time and respectfully request that our patients do the same. Consequently, we apply fees for missed appointments and for cancellations made less than 24 hours before the scheduled appointment time. The specific fees for these instances are:

• Endocrinologist consultation: 50% of consultation fee

• Endocrinologist follow-up: 50% of follow-up fee
